Palliative and end of life care
Comfort, dignity and calm at home, working alongside the district nurses and the hospice team.
Most people, asked where they would want to be, say home. Making that possible takes organisation, and a team that knows what it is and is not responsible for.
We are the hands-on care in the house: personal care, comfort, presence, and support for the family who are living through it. The clinical side stays with the district nurses, the GP and the hospice — and we make sure everyone is talking to each other.
Who this suits
- Someone who has chosen to be at home for the last months, weeks or days
- Families who want to be family rather than carers at the end
- Situations where the hospice at home team need extra hands
- Anyone needing care alongside a syringe driver or an anticipatory medication plan
When it is not the right answer. We do not give injections or manage syringe drivers — that is district nursing. What we do is make sure someone is there in between, which is usually the gap families feel most.

The care plan is written with you, so this is a starting point rather than a fixed list.
Comfort and personal care
Gentle washing, mouth care, repositioning and pressure area care, at whatever pace the day allows.
Being there
Sitting with someone so they are not alone, and so the family can sleep, eat, or go outside for ten minutes.
Working with the nurses
We record what we see and pass it on, so the district nursing team are not working blind between visits.
Support for the family
Practical help with the house and the day, and an honest answer when you ask what is happening.
Nights
Waking or sleeping nights so the family are not doing every night themselves.
After
We do not disappear the same afternoon. We help with what needs doing and we come to the funeral if you would like us there.
