For social workers, commissioners and discharge teams

Refer to us

Everything a social worker, commissioner, discharge team or case manager needs to place someone with us — what we take, how fast we answer, and how to send it over.

Our commitment

Timescales you can plan around

These are the real numbers, not aspirations. If we are going to miss one, we will ring you before we do.

  • 012 working hoursWe acknowledge your referral
  • 021 working dayA named coordinator responds
  • 032 working daysA costed proposal, or an honest no

Out of hours

Outside those hours our on-call line covers people already receiving care. New enquiries reach voicemail and we call back the next working day. In an emergency, always call 999 or 111.

What we take

Referral routes we work with

We cover Trafford, Manchester, Stockport, Cheshire East, Warrington, and we take 12 service types across visiting, live-in and complex care.

  • Local authority packages

Adult social care packages across Trafford, Cheshire East, Warrington, Manchester and Stockport, including framework and spot purchase.

  • Continuing healthcare

ICB-funded CHC packages and personal health budgets, including complex and delegated clinical tasks with clinician sign-off.

  • Hospital discharge

D2A and discharge-to-assess referrals. Ring us while the discharge date is still being discussed, not on the day.

  • Case management

Post-injury and litigation packages, working to a case manager’s plan with the reporting that requires.

  • Direct payments

Families and individuals commissioning their own support with a direct payment or personal budget.

  • Supported living

Support hours attached to a tenancy, single or shared, working alongside housing providers.

What to expect

How we work with you

The things that make a provider easy or difficult to work with, written down so you can hold us to them.

  • A named coordinator

One person who knows the case, with a direct number. You will not be re-explaining it to whoever picks up.

  • An honest yes or no

If we cannot staff it safely we say so quickly, so you are not holding a placement open on a maybe.

  • Trained before we start

For delegated clinical tasks, a clinician trains and signs off each carer on the individual. We do not start and learn.

  • Reporting that is usable

Daily notes, MAR charts, incident and safeguarding reporting in a format that stands up at a review.

  • We flag deterioration

We escalate the same day rather than waiting for a review. Early flags are cheaper than admissions.

  • Realistic capacity answers

We tell you what we can genuinely staff this week, not what we hope to recruit next month.

Send it over and we will come back to you

Use the form, or ring the office if it is urgent or a discharge is imminent. Please do not send patient-identifiable detail by email — we will set up a secure route once we have acknowledged the referral.
