The first few weeks after a discharge — the shopping in, the medication understood, the falls risk checked.
The riskiest fortnight in an older person’s year is the one straight after a hospital discharge. The house is cold, there is no food in, the medication has changed, and confidence has taken a knock.
We cover that gap. Often it is short — two or three weeks — and then it stops, because the point was to get someone back on their feet rather than to start a permanent package.
When it is not the right answer. We are not a reablement therapy service. If someone needs physiotherapy or occupational therapy input, that comes through the hospital or the community team — we support the plan, we do not replace it.
What is included
The care plan is written with you, so this is a starting point rather than a fixed list.
Heating on, bed made, milk and bread in, and the fridge cleared of whatever was in it three weeks ago.
Going through what changed, when to take it, and prompting or administering until it is habit.
Rugs, flexes, lighting, the step into the kitchen — the things that put people back in hospital.
Washing and dressing help for as long as it takes, and no longer.
Proper food and enough to drink, which is most of what recovery actually needs.
Getting you there, and making sure what was said gets written down.
For a discharge, often within 24 to 48 hours. Ring us as soon as a discharge date is being discussed rather than on the day.
